NOMINATION FORM

The Community Champion award celebrates an individual or organisation in the community who have significantly contributed to the promotion and facilitation of quality Physical Education experiences.

Eligibility Criteria:
  1. Open to any individual or group.
  2. Can be nominated by any individual or organisation.
Award Criteria:
  1. The nominee, whether an individual or organisation, must have demonstrated a strong commitment to enhancing physical education experiences within their community. This could include providing access to PE resources, supporting local schools or clubs, or organising events focused on physical education.

  2. The nominee should actively advocate for the importance of quality physical education within the community, helping to raise awareness and foster a culture that values movement, health, and lifelong physical activity. This advocacy could include public speaking, community outreach, or collaboration with other local advocates.

  3. The nominee should have shown ongoing dedication to supporting quality physical education over a sustained period. This might involve years of continuous service, long-term programming, or multiple initiatives that consistently support active lifestyles within the community.
Additional Requirements:
  • The nominee must be endorsed by at least one community leader, educator, or stakeholder who can attest to their contributions. Additional testimonials from participants, whānau, or other community members are encouraged to highlight the nominee’s influence and the positive experiences they have facilitated.
